Patients who have received care at Panola Medical Center may request copies of their medical record/health information by contacting the hospital and requesting an “Authorization for Release, Use and Disclosure of Health Records” form.

Clinic Medical Records

 

If a patient needs access to their clinic records, they should contact the clinic directly. The clinic staff will guide them through the proper process to request and obtain their medical records securely and efficiently. For privacy and security reasons, records are not released through third parties or over unsecured channels.
